What is an Issue Log?

An Issue Log is a document used to track issues and errors that occur when testing software.

In general, the issue log is used to identity issues which need to be resolved before the software is ready for production or the next phase of testing.

Purpose of an Issue Log Template

An Issue Log helps Software Test managers to:

  • Ref # – identify each unique error that occurs for traceability
  • Date Raised – enter the date when the issue was raised
  • Issue Related to – describe how this issue is related to other parts of the system
  • Description – provide as much detail as possible, including screenshots to help others determine the root cause of the issue
  • Priority – estimate whether it is low, medium, or high priority
  • Resource – identify the software tester who discovered the issue
  • Activity – describe the activity when caused the issue to occur
  • Outcome – describe the outcome as a result of this step
  • Resolved Date – enter the date when the issue was resolved
  • Status – enter the status: open, closed, in review etc.
